Warranty

The PuriLens System comes with a 90-day limited warranty against faulty materials and workmanship. This warranty does not include damage to the product resulting from accident, misuse, improper installation, or operation.

The PuriLens warranty will be extended for life provided that your patients:

1. Complete and submit their PuriLens registration card via this website, by mail, telephone or fax.
2. Purchase PuriLens Solution from an authorized PuriLens reseller.
3. Use only PuriLens Solution in their PuriLens System.
4. Complete and submit in their extended warranty card from each box of PuriLens Solution via this website, by mail, telephone or fax.
5. To validate or extend your warranty you must register your PuriLens products within 14 days from date of purchase.

Your patient's warranty will be extended for 90 days for each purchase of one-dozen (3-month supply) of PuriLens Solution. If the product should become defective within the warranty period, it will be repaired or replaced, free of charge, at the company's option. The consumer's sole remedy shall be such repair or replacement as is expressly provided above, and the company shall in no event be liable for any incidental or consequential damages arising out of the use or inability to use this product for any purpose whatsoever. Some states or provinces do not allow the exclusion or limitation of incidental or consequential damages, so the above limitation or exclusion may not apply to you. This warranty gives your patients specific rights. Thay may also have other rights.
